Job Summary

To support a growing value-based care model, the full-time Health Coach NBC-HWC Certified will provide education, behavior change support, and patient engagement for heart failure patients while collaborating with clinical teams in a remote environment.

Key Responsibilities

• Provide education and coaching on heart failure management, lifestyle modifications, and self-management techniques
• Collaborate with the clinical care team to support patient care plans and document patient engagement progress
• Respond to patient communications professionally, building trust-based relationships to enhance patient experience

Required Qualifications

• Bachelor's degree in health sciences or a related field preferred
• NBC-HWC (National Board for Health & Wellness Coaching) certification required
• Experience in healthcare, chronic disease management, or patient engagement preferred
• Cardiology or heart failure experience is a plus
• Telehealth or remote patient engagement experience preferred
