Job Summary

A company is looking for a Health Coach - Clinical Navigator.

Key Responsibilities

Provide one-on-one health coaching for patients managing chronic conditions
Utilize motivational interviewing and behavioral change techniques to guide patients
Educate patients on disease management and offer ongoing support through remote monitoring

Required Qualifications

Active health coaching certification (NBHWC, NSHC, ACE Advanced)
1+ years of experience in health coaching or patient support
Proven expertise in behavior change techniques and motivational interviewing
Experience with remote patient care or telehealth is highly desirable
Strong organizational skills and ability to work independently in a virtual environment
