Job Summary

A company is looking for a Health Navigator-1.

Key Responsibilities:
  • Conduct educational telephone calls and complete health needs assessments for members
  • Identify gaps in care, motivate members to become compliant, and assist in appointment scheduling
  • Manage system work queues, screen members for eligibility, and assign them to the clinical team for intervention

Required Qualifications:
  • Associates Degree in education, communication, or health-related field or equivalent work experience
  • 2 years of experience in a customer service support role
  • Ability to work independently or in a team environment
  • Knowledge of medical terminology
  • Experience in managing multiple tasks and setting priorities
