Job Summary

A company is looking for a Health Navigator.

Key Responsibilities:
  • Conduct educational telephone calls to advise members on benefits and services, complete health assessments, and refer to health management programs
  • Reach out to members with care gaps, motivate compliance, and assist in locating providers
  • Manage system work queues, screen members for eligibility, and assign for call outreach and intervention

Qualifications and Education:
  • Associate's Degree in education, communication, or health-related field or equivalent work experience
  • 2 years of experience in a customer service support role
  • Ability to work independently or in a team environment
  • Knowledge of medical terminology
  • Effective time management skills
