Job Summary
A company is looking for a Health Navigator to guide members in understanding their benefits and accessing resources for healthier living.
Key Responsibilities
- Conduct educational calls to inform members about available benefits and refer them to appropriate health management programs
- Reach out to members with gaps in care, assisting them in scheduling appointments and encouraging compliance
- Manage system work queues and facilitate research related to member inquiries and program processes
Required Qualifications
- Associates Degree in education, communication, or health-related field, or equivalent work experience
- 2 years of experience in a customer service support role
- Prior clinical experience in healthcare or managed care, preferably in case management or care coordination
- Experience in motivating patients towards compliance and healthy behaviors
- Familiarity with chronic disease management and preventive care strategies
