Job Summary
A company is looking for a Health Navigator to join their Member Outreach team.
Key Responsibilities
- Conduct educational calls to advise members on benefits and services, complete health needs assessments, and refer to population health programs
- Reach out to members with gaps in care, encouraging compliance and assisting with provider and appointment scheduling
- Manage system work queues, screening members for eligibility and assigning cases to the clinical team for outreach
Required Qualifications
- Associate's Degree in education, communication, health-related field, or equivalent work experience
- 2 years of experience in a customer service support role
- Ability to work independently with minimal supervision
- Knowledge of medical terminology
- Digital literacy assessment required
