Job Summary

A company is looking for a Health Navigator to join their Member Outreach team.

Key Responsibilities
  • Conduct educational calls to advise members on benefits and services, complete health needs assessments, and refer to population health programs
  • Reach out to members with gaps in care, encouraging compliance and assisting with provider and appointment scheduling
  • Manage system work queues, screening members for eligibility and assigning cases to the clinical team for outreach


Required Qualifications
  • Associate's Degree in education, communication, health-related field, or equivalent work experience
  • 2 years of experience in a customer service support role
  • Ability to work independently with minimal supervision
  • Knowledge of medical terminology
  • Digital literacy assessment required
