Job Summary
Supporting the BlueCare Plus team, the full-time remote Health Navigator will assist members in scheduling wellness visits, coordinating follow-up care, and understanding their benefits to promote healthier living.
Key responsibilities
- Conduct educational calls to inform members about available benefits and services, completing health needs assessments and referring them to appropriate programs
- Encourage members with gaps in care to become compliant by assisting with provider searches and appointment scheduling
- Manage system work queues by screening members for eligibility and coordinating outreach with the clinical team
Required qualifications
- Associate's Degree in education, communication, or a health-related field, or equivalent work experience
- At least 2 years of experience in a customer service support role
- Knowledge of medical terminology
- Ability to work independently and manage multiple tasks effectively
- Proficient interpersonal and organizational skills
