Job Summary
Supporting the BlueCare Plus Clinical Support team, the full-time remote Health Navigator will assist members in navigating their healthcare needs by answering questions, completing Health Risk Assessments, and ensuring timely support through phone interactions and administrative tasks.
Key responsibilities
- Conduct educational calls to advise members on available benefits and complete health needs assessments
- Encourage compliance by reaching out to members with identified gaps in care and assisting with provider location and appointment scheduling
- Manage system work queues and screen members for eligibility and prior case activities, assigning them to the clinical team as needed
Required qualifications
- Associate's Degree in education, communication, health-related field, or equivalent work experience
- Two years of experience in a customer service support role
- Knowledge of medical terminology
- Ability to work independently and manage multiple tasks effectively
- Proficient interpersonal and organizational skills
