Job Summary
Supporting BlueCare Plus Clinical Support, the full-time remote Health Navigator will assist members by answering healthcare questions, completing Health Risk Assessments, and ensuring timely support through phone interactions and administrative coordination.
Key responsibilities
- Conduct educational calls to advise members on available benefits and complete health needs assessments
- Reach out to members with care gaps, encouraging compliance and assisting with provider location and appointment scheduling
- Manage system work queues, screening members for eligibility and assigning cases to clinical teams for outreach
Required qualifications
- Associate's Degree in education, communication, health-related field, or equivalent work experience
- Two years of experience in a customer service support role
- Knowledge of medical terminology
- Ability to work independently and manage multiple tasks effectively
- Proficient interpersonal and organizational skills
