Health Navigator

Job Summary

Engaging daily with clients and teammates, the full-time Health Navigator will monitor medical alert alarms, assess client needs, and coordinate responses while working remotely.

Key responsibilities

• Monitor a high volume of medical alert alarms and determine appropriate response actions
• Contact clients to assess alerts, verify information, and manage inbound and outbound communications
• Maintain accurate records and assist with the enrollment and support of new and existing clients

Required qualifications

• High School Diploma required
• Two (2) plus years of related professional or healthcare medical experience
• LPN/CNA/Medical Assistant certification preferred
• Call Center experience preferred
• Proficient in Microsoft Teams, Word, Excel, and Outlook
