Job Summary

Proactively engaging members, the full-time Health Navigator will conduct outbound calls to educate them on benefits, close gaps in care, and assist with accessing healthcare resources while working remotely.

Key responsibilities
  • Conduct educational calls to inform members about available benefits and complete health needs assessments
  • Encourage members with gaps in care to comply with health recommendations and assist in scheduling appointments
  • Manage system work queues by screening members for eligibility and assigning them to clinical teams for outreach
Required qualifications
  • Associate's Degree in education, communication, health-related field, or equivalent work experience
  • Minimum of 2 years of experience in a customer service support role
  • Familiarity with medical terminology
  • Ability to work independently with minimal supervision
  • Proficient interpersonal and organizational skills
