Health Navigator

Job Summary

Supporting clinical staff in care coordination, the full-time Health Navigator will assist members in accessing medical, social, or behavioral services while working remotely.

Key responsibilities

• Provide support to participants in case and disease management programs to achieve treatment goals
• Assist members in navigating and connecting to clinical and community resources, including health screening programs
• Document care coordination activities and maintain communication with participants, caregivers, and providers

Required qualifications

• High school diploma or equivalent education, experience, or training required
• Minimum of two years of experience in an acute care facility, preferably in case management or emergency department
• Preferred certifications include Medical Assistant, CNA, or Home Health Aide
• Knowledge of managed care, including contracted and non-contracted providers
• Spoken and written fluency in English
