Health Navigator - Women's Health

Job Summary

A company is looking for a Health Navigator to support members in Women's Health and First Year of Life programs.

Key Responsibilities:

• Serve as primary contact for program members, providing education and navigation support
• Conduct outreach to engage women, pregnant members, and new parents
• Identify health and social needs; connect members to clinical teams and community resources

Required Qualifications:

• Associate's Degree in education, communication, or health-related field or equivalent work experience
• 2+ years of experience in health navigation, care coordination, or member engagement (healthcare preferred)
• Demonstrate knowledge of women's health and infant care or willingness to learn
• Ability to work independently with minimal supervision
• Proficient in medical terminology
