Health Promotion Specialist

Job Summary

A company is looking for a TEMP Health Promotion & Outreach Specialist.

Key Responsibilities

• Conduct member outreach related to health promotion and chronic condition management
• Assist members in overcoming barriers to care and document all outreach interactions accurately
• Make 50-70 outbound calls daily and collaborate with internal departments to support health promotion programs

Required Qualifications, Training, and Education

• Bachelor's degree in health promotion, community health, health administration, public health, or a similar field
• Knowledge of health promotion, public health principles, and social determinants of health
• Experience with technology applications for member outreach and documentation
• Ability to work independently with remote supervision
• Consistent and dependable attendance with adherence to schedule is critical
