Job Summary
Providing telephonic liaison support to members, the full-time Healthcare Access Navigator will identify barriers to healthy outcomes and connect members with necessary resources to achieve their healthcare goals.
Key responsibilities
- Serves as a member liaison throughout the program lifecycle, offering support and resources to enhance healthcare outcomes
- Communicates with members and caregivers to identify and address barriers to health, aiming to reduce unnecessary admissions and emergency visits
- Manages appointment scheduling and follow-ups, while documenting all interactions and case updates accurately
Required qualifications
- At least 2 years of customer service experience, preferably in a healthcare setting, or equivalent education and experience
- Working knowledge of medical/pharmacy terminology and NCQA guidelines
- Strong organizational skills with the ability to manage multiple tasks simultaneously
- Proficiency in Microsoft Office and the ability to learn new software programs
- Ability to work independently with minimal supervision while collaborating with various stakeholders
