Healthcare Advocate

Job Summary

To support a growing care navigation team, the part-time Healthcare Advocate will provide guidance and support to seniors and their families in accessing healthcare, creating individualized care plans, and connecting them with community resources, all while working remotely.

Key responsibilities

• Develop trusting relationships with patients by assessing their medical, social, and environmental needs
• Create individualized care plans and connect patients to local services, including transportation and meal delivery
• Document patient interactions and collaborate with clinical teams to optimize workflows and improve the patient experience

Required qualifications

• Associate's or Bachelor's degree in Nursing, Social Work, Public Health, Health Administration, or a related healthcare field, or a Certificate/Diploma in Practical Nursing (LVN/LPN)
• Active RN, LSW/LMSW/LCSW, CCM/ACM, CHW, LPN/LVN, or other relevant clinical or care management credential preferred
• 2+ years of experience in care navigation, patient advocacy, or related fields, preferably with older adults or Medicare populations
• Solid knowledge of Medicare rules, benefit structures, claims, and appeals
• Understanding of Social Determinants of Health (SDOH) and experience working with diverse populations
