Healthcare Advocate

Job Summary

To support a growing care navigation team, the part-time Healthcare Advocate will guide patients and their families in accessing healthcare, clarifying provider instructions, and connecting with community resources while working remotely.

Key responsibilities

• Develop trusting relationships with patients by assessing their medical, social, and environmental needs
• Create individualized care plans that integrate medical, social, and behavioral supports
• Document patient interactions and collaborate with clinical teams to enhance the patient experience

Required qualifications

• Associate's or Bachelor's degree in Nursing, Social Work, Public Health, Health Administration, or related field, or a Certificate/Diploma in Practical Nursing (LVN/LPN)
• Active RN, LSW/LMSW/LCSW, CCM/ACM, CHW, LPN/LVN, or other relevant clinical or care management credential preferred
• 2+ years of experience in care navigation, patient advocacy, or related fields, preferably with older adults or Medicare populations
• Solid knowledge of Medicare rules, benefit structures, and claims processes
• Deep understanding of Social Determinants of Health (SDOH) and experience with diverse populations
