Healthcare Authorizations Specialist

Job Summary

To support a growing telehealth practice, the full-time Authorizations and Transitions Specialist will manage partner authorizations and coordinate care transitions between agencies, ensuring a smooth patient experience while working remotely.

Key Responsibilities

• Conduct eligibility letter and respite authorization processing, maintaining accurate tracking and communication with partners
• Own the end-to-end transition process for patients moving between Home Care Agencies, ensuring timely follow-up and coordination
• Document all transition-related activities in internal systems, updating care team information and maintaining clear notes on outreach attempts

Required Qualifications

• 1-3+ years of experience in healthcare operations, care coordination, or similar roles
• Associate's Degree preferred
• Experience with referrals, authorizations, or eligibility workflows
• Familiarity with EMR systems (Athena preferred) and CRM tools (HubSpot or similar)
• Experience in home health, telehealth, or care coordination environments is preferred
