Healthcare Patient Outreach Assistant

Job Summary

To support a growing Primary Care team, the full-time Healthcare Patient Outreach Assistant will engage with patients from Urgent Care clinics to promote the establishment of primary care relationships, conducting outbound calls and scheduling appointments while initially working onsite with the potential for remote work after 90 days.

Key responsibilities

• Review daily lists of Urgent Care patients without a Primary Care Provider and conduct 30+ outbound calls per day
• Engage patients in a supportive manner to discuss their recent visits and the importance of having a dedicated PCP
• Document outreach attempts and patient interactions accurately in the EMR while collaborating with the Primary Care team

Required qualifications

• High School graduate or equivalent
• Six months of experience in a clerical office, general business, or customer service setting
