Job Summary

To enhance patient care coordination, the full-time Healthcare Services Coordinator will serve as a liaison for patient activities, manage administrative tasks for healthcare providers, and ensure effective communication of program benefits while working remotely.

Key responsibilities

• Navigate patient care by educating them on program benefits and coordinating with healthcare providers
• Document patient goals and follow up on care plans, including visit reminders and authorization statuses
• Organize patient correspondence and future plans based on interactions recorded in electronic medical records
