Job Summary

A company is looking for a Heart Transplant Coordinator II for remote on-call nights.

Key Responsibilities:

Plan and coordinate all aspects of care for heart transplant patients from referral to follow-up
Coordinate interdisciplinary team, including physicians, nurses, social workers, and support staff
Develop, implement, and evaluate educational programs/projects for patients, personnel, and the community

Qualifications:

Bachelor's of Science in Nursing required
Basic Life Support (BLS) certification required upon hire
Registered Nurse License, California required upon hire
Transplant Coordinator certification required upon hire
1 year ICU experience (remote team) and 3 years acute care experience required
