Job Summary
A physician-owned, physician-operated group of health plans and related companies has a current position open for a Remote Behavioral Health Community Care Coordinator in East St. Louis.
Core Responsibilities of this position include:
- Coordinating care for high risk behavioral health populations
- Working cooperatively with multidisciplinary care teams
- Performing appropriate assessments
Applicants must meet the following qualifications:
- BA/BS degree in social science, social work or related field
- Minimum restricted licensure or evidence of progress to independent licensure status
- One year of supervised social work or counseling experience in a health care setting, working directly with individuals, is required
- One year experience in discharge planning, home health care, community health or managed care
