A diversified managed healthcare company is searching for a person to fill their position for a Remote Care Coordinator in Phoenix, Arizona.
Core Responsibilities of this position include:
- Engaging members telephonically to coordinate services, community resources, and treatment needs
- Delivering a holistic approach to coordinated care based on the enrollee needs
- Identifying early risk factors, conduct ongoing assessments and document in an electronic file
Qualifications Include:
- Two years clinical or case management experience
- One year experience in long-term care, home health, hospice, public health, or assisted living
- Bachelor's level degree in social work or other related field or RN, LPN preferred
