Job Summary

A physician-owned, physician-operated group of health plans and related companies has an open position for a Home Based Community Care Coordinator.

Core Responsibilities of this position include:

• Serving as the direct personal contact in the community
• Acting an advocate for the member
• Developing and maintaining relationships with key individuals in the community

Qualifications Include:

• Must have BA/BS degree in social science, social work, or related field
• One year of program experience, which is defined as assessment, provision and/or authorization of formal services for the elderly, may replace one year of college education up to and including four years of experience replacing a baccalaureate degree
• Ability to possess a valid State of Illinois drivers license and to travel is required
