Job Summary

A provider of healthcare and healthcare benefits has an open position for a Home Based House Calls Nurse Practitioner in Union.

Must be able to:

• Comply with all HIPAA regulations and maintain security of protected health information (PHI)
• Conduct in-home health assessments on enrolled plan members
• Communicate findings in your assessment that will be used to inform the PCP of potential gaps in care

Qualifications Include:

• Nurse Practitioner Certification from either the ANCC/AANP
• Active un-encumbered license to practice nursing in the assigned state
• Access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area
• Minimum of one year clinical experience in highest level of education OR 3 years experience as a Registered Nurse
• Ability to navigate stairs and a variety of dwelling conditions and configurations
• Ability to work independently
