Job Summary

Healthcare consulting form is searching for a person to fill their position for a Home Based Social Work Consultant.

Candidates will be responsible for the following:

• Assisting with the completion of in-home health history interview
• Completing assessments for our Managed Care client

Position Requirements Include:

• Two to five or more years
• Two years experience in Social Work
• Ability to work independently
• Willingness to travel
• Dedicated, reliable and organized
