Job Summary

A company is looking for a Home Care Account Manager to support home care partners under the CMS GUIDE Model.

Key Responsibilities
  • Support the onboarding of new home care agencies and assist with workflow setup and training
  • Serve as the primary operational contact for assigned home care partners, maintaining regular communication and addressing inquiries
  • Monitor partner performance metrics and prepare insights for regular check-ins and internal reviews
Required Qualifications
  • Bachelor's degree in a related field or equivalent experience
  • 2-4 years of experience in home care, healthcare operations, account management, or partner support roles
  • Familiarity with home care workflows including scheduling and documentation
  • Experience supporting external partners in an operational environment
  • Comfort working with operational dashboards, KPIs, and performance reporting
