Home Health Care Coordinator

Job Summary

A company is looking for a Home Health Referral Care Coordinator.

Key Responsibilities

• Provide staffing for home health and IV therapy referrals, assisting the Intake Department as needed
• Ensure accuracy of demographic data and obtain necessary documentation to staff cases
• Negotiate rates with local vendors and manage intake referrals, letters of authorization, and document management

Required Qualifications

• Some college education and/or medical background; bilingual preferred but not required
• Experience in work comp insurance, referrals, and authorizations preferred
• Knowledge of medical terminology and moderate PC skills, particularly in MS Office
• Able to work cohesively with co-workers and external customers with minimal supervision
• Understanding of injured workers' rights and commitment to maintaining confidentiality
