Job Summary

A company is looking for a Platform Coordinator in Home Health Care.

Key Responsibilities

• Support Care Coordinators with platform referrals and process patient referrals from various systems
• Communicate with team members regarding patient updates and respond to inquiries about referral status
• Ensure compliance with HIPAA regulations and meet productivity goals set by the supervisor

Required Qualifications

• High School Diploma (or GED); Associate's or Bachelor's Degree in a related field preferred
• Ability to manage multiple tasks and work under pressure effectively
• Experience with EMR databases such as Allscripts, Navi, Epic NCH, and PowerChart
• Strong organizational and time management skills
• Proficient in Microsoft Word, Excel, and Outlook
