Home Health Care Coordinator

Job Summary

A company is looking for a Home Health Referral Care Coordinator to support their referral coordination team.

Key Responsibilities

• Coordinate home health and IV therapy referrals and assist with intake as needed
• Ensure accurate processing of referrals by obtaining necessary documentation and promoting positive relations with referral sources
• Manage inquiries and follow up with stakeholders while ensuring compliance with regulatory requirements

Required Qualifications

• Some college education and/or medical background, with LPN preferred
• Preferred experience in Work Comp Insurance, referrals, and authorizations
• Proficient in medical terminology and moderate PC skills, including MS Office
• Ability to work cohesively with coworkers and external customers with minimal supervision
• Knowledgeable of injured workers' rights and able to maintain confidentiality of data
