Home Health Care Coordinator

Job Summary

Supporting the Home Health Operations team, the full-time Home Health Referral Care Coordinator will coordinate referrals, communicate with providers and injured workers, and ensure timely access to home health and IV therapy services in a fully remote environment.

Key responsibilities:

• Manage home health and IV therapy referrals, assisting with intake and documentation
• Promote positive relations with referral sources and ensure accurate demographic data and documentation
• Coordinate with local vendors and negotiate rates for necessary services to support care coordination

Required qualifications:

• Some college education and/or medical background; LPN preferred
• Experience with Work Comp Insurance, referrals, and authorizations preferred
• Proficiency in medical terminology and moderate PC skills, including MS Office
• Ability to work collaboratively with co-workers and external customers
• Knowledge of injured workers' rights and commitment to confidentiality and privacy
