Job Summary

A company is looking for an Intake Coordinator, Home Health, Work From Home.

Key Responsibilities

• Serve as the initial point of contact for potential home health patients and manage new referrals
• Act as a liaison between patients, families, and referral sources, ensuring seamless communication
• Conduct ongoing follow-ups with patients and collaborate closely with Home Health nurses

Required Qualifications, Training, and Education

• Minimum of 5 years of telephonic experience in a healthcare setting
• 3-5 years of experience using the Epic healthcare system
• High School Diploma or GED
• 2 years of recent relevant experience
• Knowledge of medical terminology and coding systems (CPT, HCPCS, ICD-9)
