Job Summary

A company is looking for a Central Intake Coordinator for Home Health.

Key Responsibilities

• Receive and document new referrals in a timely manner
• Complete workflow tasks in the Agency's electronic medical record
• Coordinate care with nurses, social workers, and caregivers

Required Qualifications

• Graduate of an accredited school of Vocational/Practical nursing preferred; high school diploma or equivalent if non-clinical
• One year of experience as a Licensed Vocational/Practical Nurse, preferably in homecare
• Current driver's license
