Job Summary
To support patient care coordination, the full-time Home Health Scheduler Coordinator will manage scheduling for field staff using HCHB, assist with admissions and intake information, and serve as a liaison between patients and clinical personnel in a remote setting.
Key responsibilities:
- Compile and manage daily schedules for clinical staff within the HCHB workflow
- Assist in care coordination and communicate with CFSS regarding order approvals and scheduling issues
- Process visits returned from clinicians and complete daily workflow tasks
Required qualifications:
- Associate's degree preferred
- At least one year of experience in home health preferred
- At least one year of customer service experience
- Proficient in Microsoft Office Suite
- Minimum of two years of general office experience, including one year in data entry or word processing
