Job Summary
To support patient care coordination, the full-time Home Health Scheduling Coordinator will manage scheduling for field staff, assist with admissions and intake information, and communicate staffing needs while working remotely.
Key responsibilities:
- Compile daily schedules for clinical staff within the HCHB workflow structure
- Assign POD clinicians to patients and assist in care coordination as a liaison between field staff and patients
- Process visit requests and communicate with the Assistant Clinical Manager regarding workflow issues
Required qualifications:
- Associate's degree preferred
- At least one (1) year of experience in home health preferred
- At least one (1) year of customer service experience
- Proficiency in Microsoft Office suite
- Minimum of two years general office experience, including data entry or word processing
