Hospital at Home Coordinator

Job Summary

A company is looking for a Hospital at Home Coordinator (Remote - Evening & Weekend Hours).

Key Responsibilities

• Manage patient intake and service coordination for the Hospital at Home program
• Schedule skilled and non-skilled visits and verify completed services
• Communicate with clinicians and internal teams to ensure continuity of care

Qualifications

• Experience in home health care, scheduling, intake, and care coordination
• Proficiency with healthcare software systems (Kinnser and/or AlayaCare preferred)
• Ability to work independently in a remote environment
• Reliable internet connection and dedicated workspace
• Experience supporting hospital-at-home or acute home care programs is preferred
