Hospital Transfer Coordinator

Job Summary

Working remotely on a full-time basis, the Hospital Transfer Coordinator will facilitate patient transfers from internal and external facilities into the Cleveland Clinic Health System, collaborating with caregivers to optimize patient placement and maximize enterprise capacity during night shifts from 10:00pm to 6:30am, including every other weekend and holiday.

Key responsibilities

• Serve as the initial point of contact for patient transfer requests, obtaining necessary information from providers
• Coordinate referrals to appropriate medical services and connect referring parties with accepting providers
• Maintain communication with Financial Counselors, Case Managers, and nursing staff to expedite patient transfers

Required qualifications

• Associate's Degree with at least one year of relevant clinical experience, or High School Diploma/GED with two years of experience
• Knowledge of medical terminology
• Working knowledge of computerized hospital admission/bed management systems
• Strong customer service skills
