Hospital Transfer Coordinator

Job Summary

Facilitating patient transfers from various facilities, the full-time Hospital Transfer Coordinator will manage communications with providers, coordinate referrals, and optimize patient placement while working remotely during evening shifts from 2:00 PM to 10:30 PM.

Key responsibilities:

• Serve as the initial point of contact for patient transfer requests, gathering necessary demographic, insurance, and clinical data
• Coordinate with internal and external providers to ensure appropriate patient placement and follow up with relevant stakeholders
• Participate in maintaining daily systems to maximize patient flow and optimize bed capacity across the enterprise

Required qualifications:

• High School Diploma or equivalent required
• Minimum 2 years of extensive customer service or healthcare experience required
• Knowledge of medical terminology required
• One year of relevant clinical experience as a Clinical Technician, EMT-P, LPN, or RN preferred
• Strong customer service skills and familiarity with computerized hospital admission/bed management systems preferred
