Job Summary
To support a growing care practice, the part-time contract Idaho Licensed Nurse Practitioner will provide high-quality virtual care to senior patients with chronic conditions, collaborating with a transdisciplinary team to develop and implement treatment plans while ensuring comprehensive support for patients and their families.
Key responsibilities
- Manage complex chronic comorbidities and create personalized care plans in collaboration with the care team
- Monitor patient progress, adjust care plans as needed, and provide clinical supervision to Care Navigators
- Complete clinical documentation and billing for Chronic Care Management and ensure compliance with payer requirements
Required qualifications
- Current, unrestricted Advanced Registered Nurse Practitioner (ARNP) license in Idaho
- Two or more years of outpatient experience in independent practice or a larger medical center
- Experience with chronic, longitudinal care models in digital health
- Master's level ARNP with additional training or experience in Geriatric care, Dementia, and Palliative specialties
- Board certification as a Nurse Practitioner in geriatric care is preferred; DEA certification is required
