Idaho Licensed Patient Navigator

Job Summary

A company is looking for a Patient Navigator (LPN) - REMOTE.

Key Responsibilities

• Conduct timely outreach to patients following Emergency Department visits to ensure appropriate follow-up services
• Perform comprehensive clinical chart reviews to identify medically necessary network direction and recommendations
• Coordinate and monitor referrals to specialists and community resources, ensuring timely appointments and continuity of care

Required Qualifications

• Licensed Nurse in the state of Idaho (LPN) at the time of hire
• Bachelor's degree in nursing, Human Resources, Healthcare, or related field OR equivalent experience
• 3+ years in health plans, provider networks, or medical group practices
• 5 years clinical experience preferred for licensed roles
