Idaho Licensed RN Care Navigator

Job Summary

To support kidney health management, the remote Idaho Licensed RN Care Navigator will manage a caseload of members, facilitating care coordination, conducting health screenings, and collaborating with healthcare providers to ensure high-quality patient outcomes.

Key responsibilities

• Handle inbound and outbound calls, providing exceptional service to members
• Educate members on kidney health, dialysis options, and related co-morbid conditions
• Coordinate care by serving as a liaison among patients, their support networks, and healthcare providers

Required qualifications

• Active, unrestricted RN license required
• Bachelor's degree required; 6+ years of RN experience including 3+ years in case management may be considered in lieu of degree
• CCM certification preferred
• Three (3) years of experience in case management preferred
• Experience in a dialysis or transplant center preferred
