Illinois Care Management Coordinator

Job Summary

A company is looking for a Utilization / Care Management Support Coordinator II to assist with administrative care management activities.

Key Responsibilities

• Perform outreach to members to support care plan next steps and connect them to resources
• Serve as front-line support for member and provider inquiries regarding care plan procedures
• Document and maintain non-clinical member records in accordance with state and regulatory requirements

Required Qualifications

• High School diploma or GED required
• 1 - 2 years of related experience preferred
• Experience with utilization management, authorizations, or managed care is a plus
• Knowledge of local health plan activities and resources
• Ability to address Social Determinants of Health (SDOH) needs
