Job Summary

A company is looking for an IL Licensed Bilingual Clinician: LMSW, LCSW or LPC in Behavioral Health.

Key Responsibilities

• Provides customized care and treatment plans for patients within assigned Care Pods
• Coordinates behavioral health care with primary care providers and monitors patient progress
• Educates patients on behavioral health and assists in the development of self-management goals

Required Qualifications, Training, and Education

• Master's degree in Social Work, Counseling, or a related field from an accredited program
• Licensed to practice in Illinois as an associate clinician under supervision or as an independent clinician
• A secure, high-speed internet connection capable of supporting video conferencing and cloud-based systems
