Job Summary
Providing care management and population health services, the full-time Illinois Licensed Care Management RN will coordinate care for high-risk patients, manage chronic diseases, and develop personalized care plans while working remotely during daytime hours.
Key responsibilities
- Coordinate communication and care transitions between various healthcare settings and interdisciplinary teams
- Conduct holistic health assessments and provide education on medication adherence and self-care to empower patients and families
- Utilize data-driven approaches to manage patient populations and document clinical information accurately within the Electronic Medical Record (EMR)
Required qualifications
- Licensed Registered Professional Nurse (RN) in Illinois
- College Diploma in Nursing
- Certification as a Case Manager within 2 years from the Commission for Case Manager Certification (CCMC)
- Experience in nursing, particularly in care management
- Ability to work collaboratively with healthcare teams and communicate effectively with patients
