Job Summary
Engaging members in their health journey, the full-time Illinois Licensed Case Management Pharmacist will conduct clinical assessments, collaborate with healthcare providers, and promote treatment adherence in a fully remote environment.
Key responsibilities
- Conduct comprehensive clinical assessments to identify member needs and opportunities for improved outcomes
- Collaborate with providers and care teams to develop individualized care plans that prioritize member understanding and engagement
- Monitor treatment progress and intervene when gaps in care or compliance are identified
Required qualifications
- Doctor of Pharmacy (PharmD) required
- Current license to practice pharmacy required
- Minimum of 3 years of experience as a licensed pharmacist in a clinical or patient-facing role
- Additional certifications related to diabetes care and patient education preferred
- Clinical expertise in patient education and counseling
