Illinois Licensed Case Management Pharmacist

Job Summary

Engaging members in their health journey, the full-time Illinois Licensed Case Management Pharmacist will conduct clinical assessments, collaborate with healthcare providers, and promote treatment adherence in a fully remote environment.

Key responsibilities

• Conduct comprehensive clinical assessments to identify member needs and opportunities for improved outcomes
• Collaborate with providers and care teams to develop individualized care plans that prioritize member understanding and engagement
• Monitor treatment progress and intervene when gaps in care or compliance are identified

Required qualifications

• Doctor of Pharmacy (PharmD) required
• Current license to practice pharmacy required
• Minimum of 3 years of experience as a licensed pharmacist in a clinical or patient-facing role
• Additional certifications related to diabetes care and patient education preferred
• Clinical expertise in patient education and counseling
