Job Summary

A company is looking for a Case Manager Behavioral Health, Assessor Team.

Key Responsibilities

Develops and administers proactive care plans to enhance member wellness and outcomes
Conducts comprehensive assessments and evaluations of member needs using clinical tools and data
Collaborates with healthcare teams and stakeholders to facilitate care coordination and overcome barriers

Required Qualifications

3-5 years of direct clinical practice experience post-master's degree
Experience in case management and discharge planning preferred
Managed care or utilization review experience preferred
Unencumbered LCSW or LPCP in Illinois
Minimum of a Master's degree in Behavioral/Mental Health or related field
