A company is looking for a Case Manager Registered Nurse, Assessor Team.

Key Responsibilities

Required Qualifications

• Minimum 3-5 years of clinical practical experience
• Minimum 2-3 years of experience in Care Management or home health care coordination
• Must possess an active and unrestricted Registered Nurse license in Illinois
• Associates degree required; Bachelor's degree preferred
• Confidence in working independently and using virtual collaboration tools
