Job Summary

A company is looking for a Clinical Pharmacist to perform telephonic outreach to improve health quality outcomes for Medicare members.

Key Responsibilities:

• Perform telephonic outreach to enhance health quality outcomes
• Optimize pharmacy therapy through engagement with providers and pharmacies
• Oversee pharmacy projects from inception to completion

Required Qualifications:

• Bachelor's degree (or higher) in Pharmacy
• Current and unrestricted Pharmacist license in the state of Illinois
• 3+ years of experience as a licensed pharmacist
• 3+ years of patient consultation experience
• 2+ years of proficiency in MS Office and Windows-based environments
