Job Summary

A company is looking for an IL Licensed Clinician: LSW, LCSW, or LPC in Behavioral Health.

Key Responsibilities

• Provides customized care and treatment plans for patients in assigned Care Pods
• Coordinates behavioral health care with primary care providers and monitors patient progress
• Documents patient interactions and treatment recommendations in electronic health records

Required Qualifications, Training, and Education

• Master's degree in Social Work, Counseling, or a related field from an accredited program
• Licensed to practice in Illinois as an associate clinician under supervision or as an independent clinician
• Experience with evidence-based treatment approaches
• Proficiency in using electronic health record systems
• Knowledge of behavioral health screening tools
