Key responsibilities

• Conduct thorough health risk assessments and medication reviews during virtual wellness visits
• Collaborate with in-home team members to evaluate vital signs and coordinate patient care
• Provide comprehensive care management, including preventive care interventions and referrals to specialists

Required qualifications

• Board Certified as a Family Nurse Practitioner
• Active Family Nurse Practitioner License in Illinois
• Credentialed Medicaid provider for the state of Illinois
• Ability to work autonomously and demonstrate professionalism and compassion in patient care
• Technologically savvy with experience using digital tools for care coordination
